Tracy CCC
Tracy Community Connections Center stands with neighbors in Tracy who are experiencing or at risk of homelessness, meeting immediate needs through street and community outreach and guiding people into long term stability with individualized case management and housing navigation. Each year the team serves more than 1,000 individuals and families, turning urgent needs into concrete steps toward safe, permanent housing and restored dignity.
